To finalize the application, you will need to submit:

The completed application form plus essay
$50 non-refundable application fee (due with application)
$250 non-refundable registration fee (due at time of registration)
A copy of your driver's license or birth certificate
A copy of your high school diploma or GED
Two passport sized photos of yourself (for state board purposes)

Once the application has been submitted and reviewed, you will be contacted to schedule an interview with the Director as part of the approval process.
